Is there a boot/belt clip on the back of the sheath for
belt/iwb carry, or is it strictly a neck knife?
Are these things really sharp?

Neck knife only. They do come very sharp. They are really thick though and have a scandi type grind that I found was pretty easy to roll the edge. I don't think it's really meant for any kind of utility, more for defense. The tiny handle doesn't offer a very secure grip. They look really cool but something like an Izula or a Becker Necker is much more practical.

Not a neck knife only. There are enough holes and molded slits in the sheath for you to lash or strap the sheath in various ways other than just a neck knife.

yep the sheaths can be made bout anyway, they are super sharp the edges stand up to abuse it will not roll...

I have the tanto spike and the Persian spike. If you were willing to alter the sheath a bit you could carry a spike with a Spyderco G clip. The clip however would be a bit wider than the sheath, which might look a little odd, but shouldn't matter from a practical aspect.
I love the G-clip. I've already converted a number of knives, from various brands, to G-clip carry. In fact I'm going to order a few more G-clips from Spyderco tomorrow and do the conversion on my spikes. Neck knives are of dubious value , I think, unless you're somewhere you can wear them on the outside of your clothing.

yeah you can roll the edge pretty easily but they are very durable otherwise. pretty much intended for stabbing though.
