why is it called a doctor knife? what is the engraving?

in the 20's & 30's all doctors carried this pattern of knife - with it's flat rear bosters the doctors would grind pills for their clients.

Some "doctor's knife" patterns have a spatula instead of the second blade, for measuring powdered meds, etc. Also useful for pipe smokers, to scrape the pipe and pack tobacco.
