how are butt plates fastened?

Several ways I know of. You could thread a hole as you suggested. You could peen the tang over. You could do a nut on a threaded tang, like a takedown or an old Moran. My current favorite is to do the nut under the butt plate, then add the butt plate with pins to hold it in the handle material.

Dug through the pics, don't have any good ones that show exactly.
Here's one with an external nut, a turned finial in this case, threaded to fit the tang.

This particular one has the nut-under construction. I use a regular old hex nut set in a pocket in the handle material, then put the butt plate on top over that. I put three or four pins through the butt plate into the handle material. After I glue it all up, I peen the pins to hide them. This butt was put together that way. Sorry I don't have any in progress pics.

Jason, what size thread do you use? I had it in my head that a threaded tang would need to be 1/4-20 or so.

The hidden nut holds the handle on maybe with some Loc-Tite to boot. and then the butt cap is held on with the peined pins that would be epoxied into the stag.
 
Agreed, I epoxy it all together. I use the nut and some guide pins for assembly/disassembly during the shaping process.
 
In many cases I just braze a screw or bolt on the butt plate ( sometimes two of them) and drill the hole(s) in the handle to match. I epoxy the butt plate in place and grind...avoiding excess heat, of course. This works well on handles where the hidden tang does not go all the way through the handle.
 
In my case, a picture is worth a thousand words... or at least a few sentences.
