How do you wear fixed blade knives comfortably?

On my belt at the hip with a Teklok usually. For shoulder carry, I use a Mercharness for slimmer, lighter knives (Burnley Kwaiken, Sibert Flash) and have a rig from Rob Humelbaugh for a medium-sized knife (Sibert Darius). Both are comfortable. Takes a bit of fiddling and getting used to at first, but once they're set right they feel natural.
 
I carry the knife in my avatar on my belt at the 4 o'clock position all the time when I'm in town in the factory leather sheath, and its so comfortable that I'd forget I was wearing it if I wasn't used to always having a fixed blade on me. Sometimes I conceal it, and other times I don't. It has a 4 inch blade, and I've never had a problem keeping it concealed with just a t-shirt. Its not really a high-riding carry system, but its not a hanger, either. I'd consider it somewhere in between.
 
I've been known to carry a Bowie with at least a 10.5" blade, comfortably concealed with one of my Southern Comforts, while wearing shorts and a t-shirt. I'm doing a Gun & Knife Show in Lexington, KY this weekend if any doubters want to come by to see and learn. Very easy - standing, sitting, twisting.
 
I agree that horizontal carry isn't comfortable when sitting against a chairback but fine for hiking provided your backpack rides high. I just took delivery of a Kydex horizontal carry with firesteel holder for my F1 and its quite comfy to wear but you'd have to have a loose shirt tail or long jacket to cover over it if you want to keep it concealed

Ka-Bar's TDI offers multiple carry options including upside down carry and they're reasonably priced
 
I just neck carry fixed blades most of the time, mostly due to the same comfort issues you identify, and sometimes due to lack of belt (some shorts and ski pants)

However, this:
I have a Kydex sheath with a piece of para cord attached to it. The knife is inserted between my belt and pants and the cord is looped around my belt. Cross-draw. ;)
...is an intriguing idea as well. I'll have to play around with this. :thumbup:

I have a Dozier horizontal belt sheath as well, but I find it uncomfortable when bending over or crouching (and no, I don't have a big belly! :D )
