How is this handle done?

Well, it says the scales are micarta, which is available in block form. Since this is a commercially-produced knife, they may be doing the cutouts with advanced (CNC) machining methods.
However, the hobbyist maker could easily duplicate the shaping with files, sandpaper, grinders, or whatever.
The rivets may be a commercial item as well; the "blind" kind that are press-fitted together and then ground smooth.
 
Not loveless,you would see the screw in the center,most likely corbys or maybe just 1/4" pin stock.
