Once. A small Victorinox Classic SD Alox Green. Because my S-biner keychain wasn't sturdy enough apparently. Or the O-ring of the Vic too thin...
 
Sort of along the same topic..........I had my large sebenza micarta insig clipped on while I was helping a neighbor with his snow blower this year. Near the end of the session the old man found it on the driveway. Didnt know it even fell out. I would have been heart broken as that is one of the longest knives iv ever owned/one of my favorite.
 
I've been pretty lucky. I lost an Endura about 15 year ago, but that's it. Its bad enough to lose a $60 knife, but I have been carrying a Sebenza 25 lately and that would be a real bummer to lose.
 
Lose your knife! Really. Ever once in awhile I miss place one, but that's never for more than a minute or two. I'm 54, I have pocket knives from when I was 15.
 
I've only carried for a couple years and never lost one. I'm still afraid to carry a hinderer or something unless it is a special occasion just because the thought of losing it is unbearable.
 
I done diddly losted my Gayle Bradley recently. Before that, I lost my SOG Pentagon Elite like 15 years ago.
 
I've lost a good half dozen quality blades over the last 30 years or so. Main reason I stopped carrying bali's for a while.
 
I'd lost all my knives owned up through my first year of college. Not really any quality knives in the bunch, but some were special. I averaged about 1 a year.

Since then, I've lost 2 really good ones, through carelessness (no longer a problem), and one which was stolen. I did lose one last summer when out canoing, which was due to an un-planned dunking, so that's 4 in 20 years.

All recent losses have taught me lessons, so I'm confident the next few years will see minimal losses, at least until I reach a certain age.

Lessons learned:
No clip knives on slick slacks, nor loose pockets. They just don't stay put.
Tether all knives when up high, or near water.
Pocket check when leaving ANY location.
Don't travel with expensive knives.
ALWAYS make a mental note when setting a knife down.
 
Only once - it was a POS Endura-like knock-off just about 20 years ago. Cheap knife, but I was still pissed about it!

I do remember reading a story about someone finding a Sebenza just sitting in a parking lot, with some initials engraved on it, IIRC. The guy who found it was a knife guy, and knew a user when he saw one, and he either posted on craigslist or listed it in the newspaper. Got a phone call back from the original owner, (his initials) and he couldn't thank him enough. I can't remember if I saw it on here a long time ago, or somewhere else, but it's stories like that that almost make you think about engraving a phone number on the lock side if it's something like a CRK. It'd be pretty cool if you could do that on a backspacer and hope the person who finds it notices.

I'm in the same boat, mine is a Schrade 51 OT, bought back in '75

Never but recently can't find my Spyderco Dragonfly all stainless, driving me crazy !

I still got mine, ;) sorry, couldn't help myself, (now I gotta go check mine). ;)

a couple of random knives over the past 8 years (how long I've been married.) every knife I give my wife turns up missing, and any knife she decides to borrow ends up missing. never anything expensive (I know better than to buy her an expensive knife) and crkt m16 x 2. never had one go missing before august of 2005 though, lol.
You're better off without them, brother. That m16 was a disaster waiting to happen, and no knife [stamped strikethru] painted s&w is ever going to do you any favors. I almost feel like an Asian style gyuto is more your thing.

I disagree with the disaster part. the two I had did very well for $30 knives. I never used either one to baton with or anything, but I don't do that with any of my folders, but they put up with fairly hard use.
 
Lost a low buck beater in the backyard about 15 years ago. I found it the following Spring, after the 5 foot snow drift melted. That's the one and only time.
 
I was using a Spyderco Native to splice an eye in a 3" towline. The knife was issued, so everyone had one in their pocket. The knife was out and on the work bench when I got called to another area of the boat house.

I returned to no knife. The suck? I couldn't believe that one of the crew would have taken it, especially when they were issued and we all had them. I was bottom of the food chain when this happened, but I was at an awesome unit and liked everyone I worked with. I couldn't take that someone had relieved me of it. I checked pockets, every drawer in the shop, behind the counter, I cleaned the workroom. It was my first unit, my first issued knife.

Can't say if it was stolen or lost. I still hope lost, but I have that nagging feeling it wasn't.

I still drool over the Native. Have yet to pull the trigger on one with my own money even eight years down the road.

An Elishewitz mini-tank. I cried. Only one I've ever lost, but it was a nice one.
 
I lost my Benchmade 707 Sequel. I still beat myself up over it. This was at least five years ago, but I'm still a bit hopeful that it will turn up.
 
Never lost a knife, but flashlights, man I can't keep em'. I left a Streamlite in a rental car in San Antonio, I lost a foursevens preon in my move back from law school, all in the last year or two.
