That was probably me, though I wasn't the only one. The issue was the lock bar that sat slightly proud of the handle. If I remember correctly, it was only on the third run of the American version.

Dang you've been busy, I do think that Italian looks great as well and wouldn't mind having a good one.I’ve got a 2nd Gen American and an Italian.....and Elite.....half dozen Scouts. Maybe I got lucky with the Italian, but it’s flawless.
Now to find a Gen 1 American and an actual Demko 4Max, then I’ll be happy.
All the Cold Steel Triad knives I own have some minor gap between the lockbar and the liners. I'm not sure if this is tolerance issue for the Triad's function or just a cost cutting decision...probably the latter. Regardless, I would rather pay the crazy deals I've been getting and have a 4 Max at about 95% of Spyderco's quality instead of paying for Spyderco "quality" and getting an overpriced knife; I've owned Spydercos with bent blades. I recently bought three 4 Max Elites for $200 brand new and they're so much more knife than a $220 Military.I have flipped a lot of 4 maxes. Both the Italian and American had problems. Some were good and some were bad.
Some of the issues are bowed liners, gaps between the lockbar and the liners, sunken lockbar, pivot spins and cant be adjusted, terrible edge grinds, poor machining on the G10 and really bad lock-stick. Oh, and paper thin pocket clips.
Nothing against the knife. If you get a good one, its a great knife
