Preheating the mug or rinsing the filter with hot water and forget to dump it out first? I hate when I do that.

Filled to too much and walked away for a minute. I can usually catch it but not this morning.

I've done that too. Don't do it very often anymore since switching to the Clever Dripper and figuring out how much to fill it for whatever mug I'm using.
