Depends on the application. I don’t love them or hate them. I find them useful on some knives while unsightly on others. Whether they are categorized as lanyards or fobs makes no difference to me as it’s just a length of cord tied on to a knife for a specific purpose or aesthetic.

only time I use a lanyard is for my SAK Champ when I backpack; otherwise no.

When I first saw them I thought, "Why are these guys decorating their fancy letter openers?" One wife commented it was Guy's Barbie Time. I'll duck now.
 
What I call a lanyard, some purists refer to as a fob.

I like them on small knives (folders & fixed) and folders I carry in my cargo pocket (especially useful for the latter).

If it’s a medium or larger folder riding right front, no thank you.

I had a pass-around Delica for a while; the lanyard worked well for watch-pocket carry (or whatever that small pocket in jeans is called).

Always on my "water" knives, with the lanyard anchored somewhere on my person, when on, or around water; swimming, boats, piers, etc. In addition to loss prevention, you never know when you'll need to cut either yourself, or gear, loose from ensnaring nets, lines and the like. The lanyard can have a decorative, but practical function.
 
I might use them on large choppers/bushcraft knives so they don't go flying. But as an "extension" on a folder, nah not for me.

Why?
Gives me something interesting to look at on other people's knives.
Uses?
You obviously got it; for over water or up on a ladder or over snow it helps to prevent dropping the knife.
Long or short lanyards?
I like short ones on other people's knives. If I were to ever use one it would be looped to my belt or gear and would be more of a tether so . . . long.
Beads or no beads?
Beads and all manner of do das and lots of them (on other people's knives) NO SKULLS.
Type of paracord? (nano, micro, 275, 325-1, 325-3, 425, 550, 650 or battle cord?)
That's up to the user. If I were to ever use one it would be 550 because I have a bunch of that around . . . hmmmmm . . . the blue stuff ? . . . or . . . OR ! . . . the black stuff ? . . . . hmmmmm.
Anything in lieu of?
Oh sure if people want to use leather or old plastic streamers off their kids bikes . . . just anything . . . doesn't bother me. No that wasn't a challenge.

I totally get the need for tethering a knife to one's wrist while working over water or high places, but 99% of these safe queens will never see a day of work in their pampered lives.

It's pure decoration when the loop doesn't open for a hand to slip through.
 
Depends on the knife. I have fobs in my 0550 and 0566, as well as a light blue one to add color on my plain ti TiSpine. I also have one on my truly every day carry Wenger Esquire, which has its loop opposite the blade end (reverse position from the Vic Classic), so that gives me something for my ring and pinky fingers to grasp while I use the Esquire to cut tape and the like.

I’m otherwise neutral on them.
 
I can't stand lanyard holes in folders and wish none of my knives had them. Lanyard holes end up forcing clip attachment points to be too far away from the end of the handle/scales, forcing a shallow carry result instead of deep carry.
 
I've never had one installed on a folder. I'm a ninja type, mo hidden is mo betta. I thought these fobs had more of a utilitarian use. Easier to pull from ones pocket, etc. But if you have to reacquire grip from fob to knife, what's the point? Now if you have a loop where ya pinky can go through & pull, then the knife lands in your hand in a better position for acquirement, ok then.
 
If the knife comes with one, it stays on. If it doesn’t, one is not put on it.
