Looking for a pocket sheath/caddy for my EDC items.

I have been messing around with daily EDC and have recently settled on:
-Hoback ultra custom knife
-Muyshondt AeonII black flashlight
-TI2 Techliner shorty pen

All of these items are pretty small and somewhat expensive and I am worried about losing them.

I think I want some sort of sheath/caddy.
I imagine it like this:
-made out of leather or somethings.
-3.5"-4" wide with three seperate pockets sewn into it.
- slopping top edge going down from left to right so it goes tallest to smallest (pen, knife, light)
-I put my items in the sheath/caddy and then slide it into my pocket. It's wide enough to fit in and has enough bend to kind of form around my leg

Any ideas on best place to get something like this? Maybe custom leather maker? I am also go to blade next week so let me know if you think anyone there is worth checking out.

KSF Leather makes some nice pocket sheaths. I've only seen them with two pockets, one for a knife and the other for an item but they may have more options.

They're a good place to start, although custom may not be a bad route to go to get exactly what you are looking for.
