When you use ivory like this, do you leave the cracks as they are or do you fill them?
Would leaving them open be unhygienic because dirt, sweat etc can accumulate in the cracks?

I fill all cracks like that with straight CA glue--usually thin and sometimes medium gap filling, I let mine dry naturally-no accelerator-and feel this helps strengthen the base material as well, for finishing I just buff til I get the level of shine I like.
 
There is actually a product for doing what you want to do. It's called Paleo bond, and it's made for fossils. Thy have a handful of products depending on what your wanting to do. But first it needs to absorb there PB002 Penetrant Stabilizer. It's specially designed for fossil Ivory and other fragile fossil material. Thy say it has just about a zero viscosity and is designed to wick into every little crack or void and then harden. It basically stabilises the Ivory and gets it ready for filling the cracks. Then if you want to fill the cracks you would use another of there products to fill them. Thy have other high strength glues in different thicknesses depending on the size of the gaps your filling.
