Manual Folding Knife with Safety?

IMHO, trying to find a fix for a non-existent problem. Any decent slip joint won't open on its own. Get a slip joint. Carried them for 60+ years and never has one fallen open in my pocket.
 
De-assisted Benchmade Axis Assist, like the Barrage? A bit hefty but really decent
 
Smaller fixed blade and sheath?

I googled "underwear with sheath". Yeah, don't do that... 😦

Opinel 6, 7, or 8 will take you all the way. Won’t weigh your pocket down either. If someone has an issue with it you‘re only out $15 or so. And no I‘m not trying to be contrary, I carried an Opinel as my primary blade for a long time and never had an issue.
 
I've carried the Ruike P-108 with Beta Lock safety, and 14C28N blade steel, for the last three years. It's a very decent frame-lock flipper, for not a lot of cash. The Beta is a slide lock, with a relatively large slider, and very positive engagement.
Less than $40.
 
A Civivi button lock. They lock closed and they lock open.

Only the button lock Elementum does that, though the WE Double Helix (totally inappropriate for elastic waistband carry) and Kabuto (same) also function that way.
 
Get a spyderco back lock and it will be plenty safe and comfortable to operate, even fast if needed.

Opinel is in my pocket more days than not. And I always tinker with them, smoothing the opening so they swing open with the flick of a wrist.

But more importantly, have you considered a pocket sheath?
