I can recommend a few people based on my experiences.
Mike Sastre of Rivercitysheaths.com makes a lot of custom stuff for our crew. His work is awesome. I've only seen his concealed carry / quickdraw work, but it's some of the best I've ever seen.
Robert at Survivalsheath.com also makes excellent custom sheaths for more hunting and survival application. One of our guys owns a full rig he did. It's really well made.
Spen over at Jreindustries.com does some awesome custom leather options should you choose to go in that direction. I've used his sheaths before and they are great.
