I've just purchased my first neck knife. I went with the Spyderco Swick and I'd highly recommend it. This one came with a kydex sheath. I was concerned with how easily the knife came out too, so I put the heat gun to it and put a small dimple in it. It holds the knife great now and very secure. It takes a little more of a tug to get it out, but it isn't coming out unless I take it out.

If i were Tarzan, i would consider neck carry but in general i prefer belt or pocket carry. With a clip, you can atach a knife nearly everywhere.

Hmmm i occassionally carry a leatherman micra around my neck, folded up of course lol!
About 10+ years ago i carried an old Cold Steel Ready edge around my neck but the kraton tended to cling to and hangup on clothing. I transferred it to my keychain kept in my jacket pocket. That is until i had a little incident with it.
One day i was just getting home i come in through the front door and the phone was ringing so i tried to quickly take off my jacket but my watch caught up on the sleeve. Finally got the jacket off, pitched it towards the chair and ran off to answer the phone. Answered it and came back to hang up the jacket. Felt for the keys they were there but the little sheath was missing its knife. Puzzled i look around, no knife. Then my eyes stray towards the wall. there it was sticking straight into the wallboard! Very weird.
 
I have a blackie collins "buddy system" neck knife. The knife locks into the sheath. You push a button to realease it. Works fantastic. I can sleep with it on and everything.
 
I don't like things around my neck in general. I have a Becker Necker and I could see wearing it around my neck in a survival situation, but I share the same concerns of falling on the thing and getting slammed in the sternum, etc. The Kydex sheath on the Necker is tight and I have little fear of the knife falling out.

Like you, I can't stand wearing things around my neck-ever. Too much room for something go wrong. I don't even usually wear a chain (jewelry). I rigged my Becker Necker with a snap link and carry it upside down from my belt loop. If I wanted or needed to I can snap it up to other gear (like my pack) as well.

Aside from the really queer (NOT a homosexual reference) feeling of a neck knife, I periodically will do it. When I do, if concealed, it is between a t-shirt and outershirt (sweatshirt, henley, etc....). I much prefer open carry with a neck knife, but that usually freaks people out if your not in the woods or in your home. I have one with really poor sheath design. It can unitentionally be deployed, and it's popped out of the sheath more than once. Luckily no injuries. To rectify that problem I attached the lanyard to the chain and it'll help it to better stay in place.
