I've seen them done on a lathe if you have one. Of course the pilot only be a guide/pin, not drill. Although there must be a way to make it a drill too. But I ain't no machinist as you surely knows.

I know you can buy drill/counterbores in popular bolt hole sizes, try looking at major manufacturers (Guhring, Cleveland, etc.) You can also take a centercutting 2 flute end mill, and grind to size on a surface grinder. These work pretty great in a mill, probably not so much for a hand drill.
McMaster Carr is a great reference for us, but as a Canadian, we can't order from them unless we already had an account in the past.

+1 on McMaster Carr...I buy one or two about every week or so depending on what work needs to be done or what my guys break...
They also have a nice guide to match the counter bore to the material you're machining.
I didn't see anyone answer with MSC yet, but they can get you a 7/16" interchangeable-pilot counterbore and a 3/16" pilot...
I get a lot of stuff from them when they have cutting tool sales. Enco is usually cheaper, though their selection is usually more limited.
