Pitting in blade after heat treatment

I am getting some pitting and what looks like de-carbonization on my O1 tool steel blades after heat treating. I quench in canola that is warmed to 150f. Does the canola break down after repeated quenches and cause this?
 
Not really sure what you mean by pitting. I quench O1 in canola and after some light sanding to remove the scale i get this.

I have to sand down about 0.01-0.02" on each side to get a clean surface. There are grooves and a couple small dings that appear after heat treating that arent there before, at least i dont think they are.

Question #1 is - Was it forged of stock removal.
Question #2 is - How much sanding was done pre-HT?
Question #3 - How much sanding was done post-HT?
Question #4 - How did you do the HT?
