Pocket sheathes

Y'all have any good ideas for a comfortable pocket sheaths for small fixed blades?
 
A flat rectangle about 4" wide with a pouch for the blade. Holds the blade steady in the pocket and ready for use. hard to pull off with full scales, but better with flat handles.
 
How big of a knife? I like a pancake holster or stacked leather holster. A lined folding sheath with a flap is what I make for a bigger blade.
