Is holding the blade across the width of the stone acceptable when the blade is so much smaller than the width?

Also, is there any reason why you cannot grind both forward AND backwards as long as you're confident not to lose your bevel angle?

Yes, especially for a straight cutting edge. make sure your force is even across the length of the blade.

No, at the start. When you get down to the last bit, your strokes will likely become slower, lighter, & more purposeful. At that point you will just go edge leading. That's what I do.

As you have already cottoned to, bevel angle is of primary importance. Most else is a detail to be played around with, to suit ourselves.
