I wouldn't carry SOB for two reasons.
1) Aforementioned spine injuries from falls (one of our medics just slipped down a hill a few days ago into a concrete drainage ditch trying to get to an overturned vehicle) or from wrecks of our own.
2) Our district is also in a crappy area and I don't really want to provide our patients (i.e. homeless, drug addicted, prostitutes, the mentally ill etc etc) with a knife within easy reach that's harder for me to prevent them from grabbing.
If we were in a completely rural area that might be a different story.
