Thwaced it hard while screws were loose as suggested . I'm going to send it in and have Chris look at it because right now its really not safe. I'm lucky that when I found out when it disengaged my hand wasn't in the way.

It almost sounds like the lockbar has been sprung. Take the blade out and see that the lockbar contacts the opposite side with no blade in the way. It should.
