Source for a small leather job?

I've got a custom kydex sheath or my BK7 but I have no way to attach it to my belt. I'd like to have a simple belt drop with snaps made out of black leather. Basically, just a piece of leather as wide as the sheath with 4 holes for screws long enough to fold over the belt and snap to itself.

Any suggestions on where to have something like this made?
