IMHO it works best with boiling water. Just put the plastic insert in there for 30 sec and then mold it. Don't burn yourself.
I usually tighten it a bit without the knife, warm it up again, stick the knife in and mold it around the knife.

This is what I do as well and have had good results. Usually use a towel and a pair of regular pliers and needle nose pliers which work great for tight molds but does mark the kydex up some.
