I notice that a lot of makers and manufacturers use threaded stand off sleeves and screws to attach handle scales on full tang knives. How do you secure the stand off in the hole in the tang?

The stand off is typically not secured; it can freely rotate.

You need to tighten the screws from both sides at the same time when attaching the handle using this method.

Actually We are starting to do this method with our fixed blades currently instead of using Bonding agents, we use a 1/4 inch OD stand off with 8=32 threads with dommed stainless screws, we drill and ream the hole in the tang and handle then countersink to depth for the screws, tighten standoff to one side of handle, then put blade through standoffs then apply other handle side and tighten,,,, solid as it gets,,,, and Yes we use the same for a pivot in our New FRICTION FOLDERS we are in the prototype stage of completing, of course we use washers in the PIVOT on these but same manufacture process,,,,, and like I said we get them Tight as can be but also on the folders they are adjustable for tension.

PS. when grinding down the standoff we make sure on each side the standoff face resides about 50 thousandth of a inch below each handle scale hole to allow tightening so the screws don't bottom out.

Thanks, guys. I have seen some factory/mid tech knives that are set up with removable scales that appear to have the stand offs permanently affixed. I do not plan to make my scales removable because I fear that they might "remove" themselves at some point and we can't have that, so West Marine epoxy is the still order of the day.:D

Almost all of my handles are screwed on, and I have yet to have a handle come off.

A drop of Loctite won't hurt either.
