suggestions for quality small (< 3 inch) folder under 50 bucks?

There are a lot of options for this. Sandy wears scrubs daily, and a pocket clip is a necessity!
 
The smaller Ka-Bar Dozier folders are simply great tools for even less than your budget - have given a bunch of these away as thank you's in the last few years ( replaced Gerber LST's for this purpose) - the Boker/LosBanos SLB is another very civilian knife - way more useful than the size would seem to indicate.
 
Since he is in a hospital setting anything benchmade will most likely be too tactical. Never under estimate the sheepiness of sheeple. Pick non threatening like a gentlemans knife maybe a Moki or a Mcusta both of which you can find used for just a hair over the 50 dollar mark, maybe a little more for a Mcusta.
 
+1 more for the Spyderco Dragonfly, 1.2 oz, 2 & 5/16 inch vg10 steel blade with a full flat grind. Seems perfect for what he wants. If you order one online make sure you get the vg10 version not the older aus 8. you can also get it with a white FRN handle if you think the black or steel is too tacticool for a hospital. you can check them out here: White Black Stainless

I just got my white frn dragonfly, and it's a nice choice. Keep in mind, though, that the cutting edge is just 1 7/8".
