That mail sack was probably hanging on a row of sacks (surrounded by rows of other hanging sacks) and the sorter several feet away who threw it at the sack. It landed in the wrong sack and they didn't take the time to walk over and correct their mistake.
As for the Customs inspection, they were probably just curious. Wouldn't you have been if you saw knives listed on the contents?
