Its primarily use is for field appendectomies*. Just insert, twist and pull it out. Use the scissors to cut it off, then the awl to stitch the incision. (*Sterilize everything first.)

What do I do if there's no awl (because it's been replaced by that pesky hook!)?
Thanks for all the suggestions fellers, I might even occasionally use one or two of them. Would still prefer an awl though, or even the Philips head screwdriver. Maybe some people prefer the parcel-hook!
