Question What causes this.

The BF software doesn't know what to do with it. There needs to be a hosting site URL along with the file name and extension (if there is one) to display correctly. Copy the full image URL from your hosting site and use the "image" tool from the tool bar to insert the pic.

See where the mouse pointer is, pointing at the little image. click that.
then post a link to your image into the image url box above that.

make sure the image you are posting is from an image posting service.
here are some examples
http://postimages.org/
and
http://imgur.com/

make sure you copy the direct link. so it will look something like this
https://s26.postimg.org/h5bfbkc6x/2017-08-30_15_18_32-_Question_-_What_causes_this.png

it will have a jpg, png or something at the end and an http at the beginning.
