What happened to this blade when it was sharpened?

A bad grind can be many things. It could be different angles on either side, it could be different heights of primary bevels on either side, uneven grind thickness, uneven distal taper, heck some knife makers don't even do any distal taper on their knives when It's a grind shape that should have distal taper.
Gotcha, thanks. it something that the sharpener should have noticed? Could it have been avoided?
 
I did it to my Rex 45 spyderco recently. It's not as bad n I was trying to fix the factory grind. I did it myself though and know what I have to do to fix it.

If the final bevel was really bad it might be something that was the best option. I saw it happening n could have fixed it. It would have just been unnecessary edge removal though. As I use her n sharpen her it'll slowly go back to normal.
