What's the highest quality folder for $200?

I've changed my mind. The 0300 is still fantastic and a showcase of engineering, but if we're looking for a real work of art out of a knife, an assertion I find plausibly derived from your original post, then I'd suggest these three:

I've had four different models of ZT folders and they've all been great. But when I play with the obviously Ken Oniony ones (I believe the 0400 was also a KO, but it's much less obvious) I end up just wishing I was using one of those three up there.

What is that first knife?
 
you might be able to get a rexford folder for 200 and im not just saying that because he is my cousin in law
 
I would have to say MCusta. i have one of the Ken Onion Kershaw Bumps, S30V, mine is green # 1344. nice knife, great grind. That is the first blue one i have seen.

You, sir, are my twin, in knife preferences! You about blew me out of the water, with this post. I would have listed each of the 3 you did, and in that order!

With the addition of a newly aquired Spyderco Ti Millie in S30V, and (when available for $190-200 10 years ago) the Microtech LCC D/A or Manual.

FYI, to answer the question above, the Blue knife is a Kershaw Blue Bump with Titanium frame lock, and SpeedSafe assist mechanism designed by Ken Onion. 2nd: Tyrade, also Ken Onion design, and 3rd: Offset, again, Ken Onion design.
