The sheath has the extra loop that looks like it could be for a combo knife sheath like this.
The size and looks of it reminds me of one of the kids sets, or scout sets. Maybe not the Kit Karson but something similar to this.

I have the Black beauty but I missed my opportunity to get the Kit Karson.

The head is similar to some of the early Craftsman mini hatchets made by Vaughan, but the poll is a little bit more square on this one.
It definitely looks like it was probably produced for or in conjunction with a knife company as part of a combo set.
Could be a knife company or a department store, honestly it's anyone's guess.
I know imperial offered a mini hatchet in a combo set which of course they didn't manufacture themselves, I've only seen sketched catalog pictures though and never an actual photo so I don't know what they really look like.
I don't use the word often, but I say those sets are pretty rare.
This probably isn't from an imperial set, but definitely something similar.
If only it still had whatever knife came with it.
