This all supposedly stems from a guy on XDtalk who was having double taps (and that issue was never really followed up on from what I recall).
A month or so later (April iirc), another guy there said he had his 9mm go full-auto for 7 rounds when he dropped the slide at the range
, and showed some pics of the aftermath. He never said what Springfield finally told him, but the recall came about shortly after they'd told him they were a week away from knowing what went wrong with his gun.
Nobody was hurt, but he hit his range bag with one of the rounds. It is worth noting that he'd been having trigger problems with his, and had already sent it in once for that issue. From what I remember reading, the trigger wouldn't move forward when he loaded the gun and put the slide into battery, which to me seems to be a pretty big flag that something was wrong. He claims the gun was stock, and had no trigger mods, but I'm not completely convinced.
