Comes with box and pouch and sheath, as pictured. I actually only just recently bought this knife, but now I need the cash. I bought it secondhand, as a very well-cared-for knife... and that certainly seems to have been the case!
I flipped it for maybe a total of about 15 minutes, never over a hard surface. The titanium *ping!* sound when the latch smacks into the handle just right is absolutely lovely. The blade itself is about as sharp as the wit of Ricky Gervais. Just ridiculous.
It has a bit of marking from the tang pins hitting during flipping. Also some wear in the area of the latch, as you'd expect.
