Other than a BK11 sheath wearing out on me after a long and brutal 6 years, I've never had retention problems with another Becker sheath. Also, I only use one button strap. I wore a BK5 daily for a long time, ran up and down a mountain with it on (MANY times), hunted, dragged my least favorite dog out from under a 26 foot trailer over and over, fell asleep in odd places, fell out of my truck bed, carved numerous trails throughout the year, and worked 14 plus hours a day, 6 days a week for over a year and never once had it fall out of its sheath. I even left that one strap unbuttoned most of the day because I drew the knife so many times per day, it was a real hindrance to have it buttoned in. I might have been worried about it more though had I done cartwheels every day...
I will say that I find lanyards to be more of a hindrance than a benefit. My lanyard on my BK5 regularly got caught up on sticks and branches. I broke it before it unsheathed my knife though. I did not replace it.
I think a sheath is just like any other tool...intended use must be defined before you can find the right sheath or make the correct modifications.
