Most Comfortable Handle..............

Mini-grip after I sanded the molded ridges at the bottom of the handle. I hate jimping gimping, whatever it is.
 
I see I'm not the first to mention either of these, but:

Caly 3.5 for a folder

Scrapper 5 for a fixed
 
For me in the folder department it's my 1st generation Cold Steel American Lawman followed by my Spyderco Manix 2:thumbup: Fixed blade category goes to My Izula II & I have fairly good sized hands:)

Fixed for me is mora clipper and the buck nighthawk (which is odd because I own a couple of much more expensive knives) Folders, I like my large sod buster.
 
As far as folders, I'd have to say the Manix 2 is like sitting on a cushy couch for your hand.
 
I have several that I customized for my hands. Never had a production knife that was "perfect" without at least some tweaking.
 
Tad Lynch Turkey Mountain Chopper....It fits like he had my hand on site when he made the knife. Exceptional!!!

KSF's Scagel Medium Hunter also one of the most comfortable knives I've ever handled.

Favorite most comfortable folder handle is my Lone Wolf T-3.

I have large hands and like hand filling handles.
 
I can't narrow it down to just one but my short list goes like this:
#1 Buck 110
#2 Mora HighQ
#3 Spyderco Native
#4 Full sized trapper pattern slip-joint

The most comfortable handle I've ever felt was on a Benchmade 520 presidio. Comfy as anything in just about every grip. Machined aluminium to boot - just about my favourite folder handle material.
 
Folders: My favorite handle is on my XM-18 3.5" More comfy than the larger 24 and way more comfy than a lg Sebenza which sticks in the heel of my palm. I kinda like the feel of any good knife in my hand too be honest but that's my favorite.
 
For me the BArk River Bravo 1 has the most comfortable handle. Nice and fat and just feel great
 
Spyderco's have always ruled the roost in this department for me. My most comfy folder for has always been the Chinook II/III, just a pity the blade shape doesn't suit my needs too well. It seems nearly anything Ed Schempp or Sal designs suits my hands well.
