Top blade: A fleam? Veterinary?
Neither ... tho a veterinary (or s/he's secretary/receptionist) might use one.
From the days when dip and fountain pens were "king".
It is an "ink eraser" used to scrape an "oops" (includes blobs) off the paper or parchment.
Back then most - if not all - cutlery companies made them.

I want to get one, since my pen of choice is a fountain pen.
