Speaking of the ZT 0850 It drives me crazy there’s no lanyard hole, have you come across and lanyard hacks or an aftermarket clip to attach a lanyard too?
 
Another example of when a knife doesn't have a lanyard attaching point, this is a Kizer Invictus folder and the pocket clip reduces down at the very top of the bend so that the lanyard, when held in place with a whipping knot, won't slip down the side of the clip. When in the pocket the lanyard folds down and works out just great to retrieve the knife.

While not beautiful like many contributions to this thread, this simple paracord lanyard is very functional. This is my 5 day a week EDU (every day user). I will probably contract some sort of disease as it has never been washed.

Recieved a Pro-Tech Emerson CQC-7 today and the lanyard hole is the smallest I've come across. Any recommendations on cord?

Nylon mason's line perhaps? I like the braided line, vs the twisted version. Not likely to be very stealth, since colors tend to be bright neon shades of pink, yellow, orange, or green to be more visible on job sites.

To acquire bulk for an extraction fob, a bead could be used. Lots of online bead options in diverse styles exist. For cheap, check out local hobby stores, garage/yard sales, flea markets, thrift stores, etc for items that could work.
 
I came across a super small lanyard attachment point on a set of AWT scales I put on my BM Redoubt...I can't even fit a single 550 paracord into the gap! I haven't tried the cut and peel method yet but it is tight! I might have to get some smaller cord...
 
Typically my lanyards are fairly close to the end of the knife handle, that's on knives with clips, or fixed blades. On the traditional folder I leave the first knot a little farther out so that it can drape out of the pocket for retrieval. But I will still use the whipping knot close to the end of the handle so the lanyard can't slip over the end and be in the path of the blade, just safer.
Here's an example of that on this nice Brother 1512 folder that just arrived today, nice inexpensive knife that is well made.

Here you can see how the whipping knot prevents the lanyard from sliding around the end of the handle
