Looks like just the one. Blackjack started producing Becker knives in 1991 and they went bankrupt in 1997. I would guess it was made around the same time as the black coated divtul's for the FBI. The order form posted above says 1994 so that would be my guess.

Had an update on this. I uncovered something at Ethan's last time I was there in April that I forgot to add to this.

The last piece of uncertainty was the laser etching. Well I found an old blackjack necker proto at Ethan's with laser etching: even in the same font.

There was actually another blackjack neck knife proto as well as the backpacker.

If I’m recalling what Ethan told me properly this next one was supposed to come in a stacked leather neck sheath with that fork. It was meant to be handy fork and knife kit carried around the neck or in the pocket.
