I think for most people the p365 xl is the best option. For me, I prefer the p365 x macro though.
I would suggest handling them at your local shop. There are so many flavors available. It’s not the most friendly model for mag changes with my hand.

I’m still happy with my 365X bc of the versatility. Can carry super compact with a 12 rd mag, or a smidge longer grip w/17 rd mag (or just as a backup mag. Pic below). That said, I probably would have bought the x-macro if I’d waited. (Some folks will say no to comp’d carry pistols for a variety of reasons.)

Whichever one fits your hands best. I started off with the original with 10-12rd mags and experimented with them since. I’m my mind any version of the Macro is it, fits my hand well plus 17rds (35 sidecar appendix 🤣)

The Fuse is cool but for a carry gun I md be changing the fiber optic out

Sig did some Tac pack/ special bundles with the Macro recently. 4-5 mags, savior pistol pouch, maybe something else I can’t recall and it’s a pretty solid value

Or with the aftermarket support just buy a FCU from Sig or someone who parted out a 365 and build one to suit you

I carry a P365 SAS. You either love them or hate them. I have a xmacro without comp. I'm going to get a 365X and install optic for carry. The older I get the optic makes follow up shots much quicker. To quote a buddy of mine who is a NC SBI special agent (whatever that means) when he first got an optic and started training with it he said "it feels like cheating". They now just got issued G19 with RMR HD. My SAS I can pocket carry in a pocket holster if I want.
I chose the xmacro without the comp as well. It's a great carry weapon.
 
I started my career in LE carrying a S&W Model 15, 36 and finally a 19. The only one I have anymore is a 642-1 which gets carried daily around home and walking distances. It's accurate enough for what I want it for, and easy to carry.

Once I leave home in the 4Runner, it's usually a G26 with an extra mag.

Something to be said for the marriage of portability and accuracy.

That’s a pretty sweet pistol I won’t lie now. I was skeptical but not having a hammer is no big deal and carrying it should be easy. It shoots decent enough for what it’s meant for. I’m going to pick up an ankle holster for the one I just got from my mother in law.

You can try pocket carry or AIWB or IWB if you want to have a better chance of getting to your gun should you need to.

(I carried on my ankle for years, but be cognizant of the drawbacks. The hammer is not exposed, but it has one.)

Only reason I am going to try ankle carry E is it will be a backup. I might decide to just get a iwb holster for it at some point to. I don’t think I’d want to carry that as my primary CCW when I’m so use to my Shield, and the accuracy.

Perfect as a backup. Be aware, since you have scoliosis / back issues...(I have scoliosis as well but didn't know untiI I took a police physical in my twenties), that the imbalance when carrying on the ankle can cause issues for some. It never really bothered me, but something to think about. I even switched from a J frame to a G26 as my ankle gun while still gainfully employed.

(My days of carrying two guns are pretty much behind me.)
 
Got lucky and picked up two local pre-Freedom Group 870 Police Magnum trade ins for a song. Refinished the walnut on one, spruced up the SpeedFeed furniture on the other and put a Choate side folder with some other tactical goodies on for fun. Featuring my FK3 while I wait for the other preorders...
