Nice job.
A couple things. First, that's a grub hoe. You can tell by the width. Second, were it an adze you'd want some room in the eye front to back. An adze need to be tuned to present the blade to the work properly. This is done with a shim placed either in the front or back of the eye as required to tune the tool.
Here is an example of an adze tuned with a shim.

This is my yard sale find from last weekend. It is a Kelly Flint Edge weighing 5 lb. I believe this is Kentucky pattern? Handle is a House Handle octagonal.

You did a very nice job on this. That head looks to have been very well cared for, which is a real prize. By the 1950s there weren't a lot of different axe patterns still in production and I suspect that TT-Kelly would have marketed this as a Jersey. But I do stand to be corrected, the resemblance to a Kentucky is there.
I would consider this to be a Jersey.
